Geriatric Physician Services affiliated with Hartford Hospital provides office-based Primary Care and consultation for adults over the age of 60.

Our Geriatric Evaluation (Assessment) and Management Centers in three locations offer convenient access to patient and family centered care based on each individual's functional and psychosocial situation. With a staff of highly skilled experts, Hartford Hospital's Outpatient Geriatric Services address the complex needs of elderly patients with specific attention to dementia, memory impairment, frailty, and fall injuries.

Our interdisciplinary team, which includes specialists in geriatric medicine, geriatric psychiatry, rehabilitation medicine, geriatric nursing, and social work, develops a comprehensive, individualized care plan for each patient.

Geriatric and Adult Psychiatry

Geriatric and Adult Psychiatry at the Institute of Living is an outpatient practice staffed by geriatric psychiatrists, advanced practice registered nurses and licensed clinical social workers. The practice specializes in consultation, assessment and treatment of psychological and emotional disorders associated with aging. These include but are not limited to depression and mood disorders, dementias, and interpersonal and intergenerational issues.
